  9. I have bcbs of michigan and I live in Florida. Since my BMI is greater than 50 the documentation of 5 months in a weight loss plan has been waived. I have to do a psych eval, nutrional eval, egd, 2yrs weight history, and lab work. Wondering if anyone else had this experience and how soon they got the approval??
