  1. Yes they will ask for a fax number to fax all the information to your surgeons office
  2. Yes, I completed my evaluation with them. Everything went well conducted everything over the phone. They also take insurance, if I remember correctly all I paid was $30, which was good. Didn't need to spend 200+ dollars for someone to tell me I'm not crazy just overweight.. lol!!!
  3. That's great, I'm actually still in the process of completing my 6 months process, which will be up in Nov. I haven't meet with a surgeon yet, however I'm completing my process in flowood as well at merit health. Why are you having to go to Monroe? If I may ask?? And did you quiet smoking on your own, cold turkey?? No vaping???
  4. Hello Sarabell, I'm from the Jackson area, but unfortunately I'm just starting my process!! I have my first appointment tomorrow to meet with the NUT and etc.... Congrats on the start of your journey, how are things?? Was your process hard? I have a ton of questions.
