  1. Actually I believe 211 would be the lowest you could go to stay at bmi 35 and above for approval.
  2. I called on a place in Dallas and they said they could get me in for surgery in as soon as 2-3 weeks. It also doesn't hurt to ask if things are actually required or just offered if you want it. For example I called another doctor in the town where I live and they were talking about getting me in to the next seminar and when I asked if that was required before seeing the Doc the answer was actually no and I was able to get in to see him for a consultation the next day. Good luck!

  4. It's to prevent a deficiency. I'm not sure about the calcium, but I was told by the Dr that b12 attaches to food and is carried into the intestine to be absorbed and so when you are eating drastically less there is a risk of b12 deficiency and supplementing this easily prevents that possibility (which can lead to bad stuff).

  6. Prices in Texas (Dallas/Houston) range from 7800 to 12.5k. If you google for gastric sleeve houston or dallas most of the docs have the price on their site. Many include hotel stays as well as the hospital stay. There is a thread regarding this topic under the Surgeons forum, I think the title is something like "Self-Pay Help!". There were many responses with names, locations and prices there. Good luck! ETA: I think that for the area (NE) the Doc you found in Jersey is quite reasonable and at least within driving distance from where you/she lives.
