  5. I go to Dr. Richard A. Benavides at 7920 Beltline Road Suite 310 Dallas, Tx., 972-331-8100. He is just above Richardson. He put my lapband in and I go to an Aftercare Center in Richardson which he uses. I've had no problems and many come from out of state to see him when they have problems. Aurora
