    I ran into the same problem... My PCP said he shouldn't have to send a letter until it was time for my pre-op testing. i called his office first every week, then every couple of days, then daily and finally the insurance coordinator from my surgeons office called and he finally wrote and faxed the letter. Insurance approval came through days later and my surgery is scheduled for Dec. 4th. I'm so excited!!!!
