Just wondering if anyone else has this & if so how hard was it to get approved?

I don't have OH, I have Anthem BCBS of MO, and sent in all my paperwork on January 3 of 09, and was approved two weeks later. Their requirements are far less than Cigna, who I had through 2008. No 6 month Dr. supervised diet, but they do look for a 5 year weight and diet history and a letter from your physician. Your surgeon should be able to tell you all the requirements.
