    I have BCBSIL and I am still in he proces so keep in mind it may be different, but I was required to have a psychological evaluation and a 3-month supervised pre-op diet. Good Luck!

    i have bcbs anthem and live in california mine was bmi of 40 with no comorbidities or bmi of 35 with 2 comorbidities you can call customer service and ask if your plan covers WLS and what the requirements are?
