TRS Active Care 3

Has anyone used TRS Active Care 3 to cover their lap band surgery. If so what were the items that they required you to submit in order to be approved.

I used it. But, I was BMI 47 with every qualifying condition listed. Sleep apnea, high b/p, diabetes type II, even sec. hypertension of the lungs and so on. I had to give the past 4 or 5 yrs. proof from pulmonologist showing I had seen a doc and what my weight was in that time. I had to do 6 more months of 'diet' under care of PPO. They approved me..paid like clockwork (I only lacked 1700 of my out of pocket) and here I am after March 12, 09 surgery, -46 and many inches. Still losing and normal everything...no meds since day before surgery! Hang in there!
