For those of you who had your insurance approved to cover your surgery - did you have to pay your copay/coinsurance amount up front or was the part that you were responsible for billed to you after surgery?

I was asked to pay a portion upfront (they gave me a dollar amount) but I explained that I was using a credit card to pay off my TOTAL out of pocket and the larger the amount I used the better special rate I received (my CC company had a promotion going on). They didn't argue, so I paid everything once I got the bill.
