I have that and I wasn't able to get vitamins/supplements covered. They also required me to do 3 mos of nutrition counseling prior to surgery. I was also required to go to an approved institution for the surgery to have the surgery covered. There are only 9 places in Illinois.

I'm required to complete 90 days of the program which includes 2 nitrition appointments, psych eval, no weight gain among other requirements. What do you mean by "approved instution"? Do you mean the hospital must be approved?

Yes. My plan will only allow the surgery to be at specific hospitals with a higher success rate. So that being said, I found a doctor did all the tests and appointments only to find out the hospital wasn't on the approved list and essentially had to transfer all records to a new surgeon who could do the surgery at one of the 9 listed hospitals. Since every policy is different, I would look and see if you also have this requirement. I was so upset when this sprung up 1 week before surgery.
