Frustrated an Impatient

I had to have my band removed Feb 2015 due to a slip and I want to get my sleeve but idk if my insurance will cover it, if I still qualify and just other questions. I'm having to do all this under a different branch of my insurance in a different state due to a military move and under a completely different branch of a military hospital. Plus my BMI is under 35 so idk know if I even have a chance even though my medical records can show I have been gaining steadily since the removal.

Has anyone had a year or more between conversion?
