Illinois medicaid question

So I have Illinois medicaid and am in the process of my 6 month physician supervised diet. I don't feel like my doctor is documenting my attempts at weight loss very well. She more or less writes weight loss of how ever much and then continue exercise and portion control. That's it. Will Illinois still approve me with this type of documentation?

I have Ohio Medicaid and had my Dr document medical diet too. The key was to see them every 30 days for weight loss recorded and notes that you talked about weight loss or diet..that can included food choices and portion control.. The notes were not really detailed... Weight recorded why I was there weight loss and sometimes it had notes about the topic... This help count toward the 6 months I had 3 with my Dr. And 3 with Nutrition from program.

Keep in mind that these physicians work with the insurance companies daily and know what to submit to be approved, they don't get paid if you don't have the surgery. Trust the system and your choice of physicians :-)
