Myocel product label???

I am trying to get some different protien drinks to try and noticed my Dr's office sells Myocel. I e-mailed the company because I was unable to find the product label information on their web site.

If anyone has this and can scan it or type it up for me I would GREATLY appreciate it! Also how was the taste, ect?
