    Luckily I had found this link at the beginning of my research. However there are points that are vague and I was astonished that I could not get any clarification from anyone at Aetna. Thank goodness the billing person at my surgeons office new the answers right off. ( regarding the two year weigh ins..you only need one from each year.)
