Assistance Needed

I have been fighting with federal bc/bs for a year and a half trying to get them to approve me for lapband surgery. They have approved me for the bypass surgery, but I don't that one.

If any of you have had to submit a letter to bc/bs to encourgae them to approve the lapband, please send me a sample of your letter. I am just about at the end of my wits with them.
