I have Aetna im submitting my claim this week. I had to go thru 4-6 months of nutrition they can submit after 4 months. I had phyc eval had to get last 2 years weight history from primary thats was basically it.

I was just approved by Aetna, and I don't have any "hoops" to jump through. Routine bloodwork and such. There are numerous criteria that a patient has to meet for approval, so maybe that is what you mean by hoops? It can be frustrating, hang in there!
