If you had any doctors (primary, gynecologist, etc.) where you used to live call and ask them to fax a letter giving your weights. I'f you have gone to weight watchers or any where else you tracked your weight, get records from there.

I have those from where I lived before but they would be from 2011. I moved March of 2012 Should I get my stuff from my old state even though they asked for 2012 and 2013?

Give them copies of what ever you have. If they won't accept it they will let you know. This particular requirement is utterly ridiculous.

Okay I will request prior years also, thanks for the responses. My first consultation was yesterday so I'm just beginning the process and am trying my best not to miss any steps.

Great news I actually went to my ob one final time 03/02/2012 I moved 03/21/2012 so I do have a weight recording for 2012 and 2013. 2 year weight History requirement....... COMPLETE

I did not have consecutive years of documented weights, but they accepted them as I was able to provide 3 years.

I had to provide 5 years, only had 4, but for that last year, they accepted a letter from a family member as a reference acknowledging my ongoing weight problems. And it worked, got the approval today.

Sassy I do not have a date yet I'm actually just getting started. My consultation was on 01/28 lab work is done, weight History is done, my psych eval is 02/10 and 1st of 6 supervised diet appointments is 02/25 so 6 months out everything should be done by July so I'm hoping for an August/September date.
