insurance may deny???

I have highmark blue cross blue shield and and doing my 6 months diet training. This is my 3rd weigh in but my 4th month. My surgery is scheduled for Dec 19th. And from march to July I lost 25 lbs but that was before my six months started. From July to Augi lost 9 and Aug to Sept was 4. Today is my next appointment but I think I either stayed the same or maybe gained a pound. My question is could my insurance deny my surgery if I didn't lose this month?
