documentation of failed attempts

I am really sorry if this has been asked before, I searched but I still need some help. Up until now I have been going on the assumption that I would be a self-pay because of an employer exclusion. We just received our open enrollment packets for 2007 and low and behold the gods have shined upon Missouri state employees! :clap2: Qualifying criteria includes “documentation of at least two failed attempts at weight loss with a duration of at least six months each”. I know this is going to sound stupid, but I have failed many times but never documented these failures for prosperity so I am going to have to recreate these events. Can anyone give me suggestions on what the insurance company (Mercy) will be looking for in these documents?

I would call your insurance company and ask them what the criteria is. It may be that you had to have been under a doctor supervised diet, or used prescription diet pills. Or have been under the guidance of a dietician. You'll only know what your insurance company's criteria is if you call them.
