I have cigna and my plan required me to have 6 months of documented diet and exercise by my PCP. plans vary so you may just want to call them. they also required me to have a bmi of 39 with a comorbidity. i hope this helps.

I have attached Cigna's Bariatric policy. My work is changing from Humana to Cigna and I just called them last week to talk to them about WLS since I just started my 6 month physician assisted weight management program in Aug.
