Just been approved funding

Hi my doctor call last night to say I have been approved funding. Now all I need to do it fill out the forms my doctor is sending me and get aproval from the hospital (st Richards Chichister). Does anyone no what the forms are I need to fill out and what the meeting at the hospital is going to be like?
