  3. no, they do not go by starting weight.. my doctor said that even though my surgery has been okeyed, if i go before a 40 bmi they will not do my surgery.. and your bmi has to be a 35 if you have diabetes, sleep apnea, high b.p. and so on.. ive worked in doctors offices that specialize in this, and most doctors and insurances will go by the guidelines..
