  4. Hi, my name is Leah. I am 31 years old, weigh 230lbs and am 5'3" tall. I have a family history of heart disease, high blood pressure, anxiety, and depression. I suffer from PCOS, and with it mild depression and anxiety. I go for my nutrition, exercise, and mental health evaluation tomorrow(7/9). My diet and exercise will last a combined 2 hours. My mental health evaluation is an hour and is with a licensed social worker. Is there anything that I should know about before going in for my evaluations? I really want to be successful. From what I was told, as soon as my evaluations are through my paperwork will be sent to my insurance company. The scheduling coordinator told me that my surgery would be beginning to middle of August. Does that mean that I just have to go through the evaluations and then I should be approved? I called my insurance company and as far as approval goes, I just need to have a BMI of 40 or 35 with comorbidity. The surgeon told me that I am 100 pounds overweight and he discussed that I would benefit from the lap band. The nurse manager at the office also told me that I am an ideal candidate. I feel that I am ready for a life change, and I'm just nervous that something will happen and I won't be able to have the surgery. If anyone has gone through what I have felt, please let me know.
