  1. If you go to google and type Dr. Albert Gilman, there is a woman who was banded by him in 2003 or 04 and she has before after pics and she looks great. He is the only one nearest me who accepts medicaid. Now, I was wondering, how do you go about getting an appointment or knowing your insurance will cover it and if you even qualify? (I.E. My BMI is 37.8, I have asthma, knee, ankle, back problems, I'm stage 2 hypertension also.) Do I need a doctor's refferal? or do i just call? Let me know, I'm really new to all this.
