    I am trying to do this now for the 200 rewards points. I asked for help and was told ; click username to access user menu at top right of the page. select the My Surgery link then select Surgeon on the left side. Choose your surgeon. Then he said once you select your surgeon the review box will appear. I had to write my Dr's name in however the review box never did appear.
