    Hi all I live in Michigan and am in the process of getting approved for lapband. The hospital that I am going to gave me a sample letter to give to my dr so they can fill in the blanks. I have BCBS of MI PPO and all I needed was a psych eval - which insurance paid for and a letter my my weight loss dr. i do not go to a pcp. i just to to the family clinic for yearlies..etc. ARe you sure you need to get medical rec's? I guess all insurances are different. But the letter from my weight loss dr just said that she has treated me for over 5 years, which is true and that I have been on the weight loss program on and off for those 5 years, the longest consistent time was 12 months. The hospital said this letter was ok. let me know if you want to see it, I can email it to anyone. thanks
