I went through Capital One Healthcare finance or cosmetic fee plan http://www.capitalonehealthcarefinance.com/ I applied over the phone and it was very simple. They told me immediately that I was approved. They did have a pretty high interest rate at 13.9% for a 5 year plan and I had pretty good credit. Also, I did not have to send anything "overnight". I did fax them my signature on one of their documents, but that was not required...they said I could send it through regular mail. Also, if you really wanted to fax it, I'm sure your doctors office would do it if you don't have a fax available. Hope this helps, Kim
