I have UHC and am just waiting to hear back from them. Doctors office submitted the medical necessity on 8-4-16. I had to have 6 months medical supervised weight loss, psych evaluation, join their resources program, and bmi of 40 or more unless I have 2 or more comorbity. I have type 2, blood pressure and borderline sleep apnea.
