**This invitation to participate in our research study was approved by ADMIN Alex Brecher on May 22/16** Greetings, My name is Rebecca Liu and I am a PhD student at Western University in London, Ontario Canada. My supervisor, Dr. Jennifer Irwin and I would like to invite you to participate in our research study. As a bariatric surgery candidate or recipient, we want to know your thoughts on the support-related services that you are receiving before, during, and after surgery to support your long-term weight management (i.e., physiologically, psychosocially, and emotionally in the short- and long-term of the bariatric surgery process). Your perspectives will provide insight as to what still needs to be addressed to advance bariatric care worldwide. The survey will consist of a series of closed- and open- ended questions. All information given will be kept confidential.
