    I also am on HIP Plus Anthem first you have to get a referral from you primary doctor. They make the referral and the bariatric center calls to set up an appointment o meet with a nurse to go over the surgeries they provide and what all goes along with each surgery. This appointment is required. I know this because I just had my appointment with the nurse yesterday. If you are than still interested they set up an appointment with the surgeon and the dietitian. HIP requires : 180 days between your consult with the doctor and you pre op appointment 6 consecutive months Supervised weight loss and dietitian appointments food and exercise journal that you keep on your own Bariatric Psych Evaluation Labs : TSH, Nicotine, Urine, Drug Screen, HGB, and AIC this is all the information I received at my visit yesterday
