I was told I needed weights recorded from anytime in the last 5 years from any doctor. I filled the form out myself and faxed it back. Now they're saying the form needs to be signed and more recent. Any tips on how to get this form filled out ? I don't regularly see my PCP but have seen my GYN a lot in the past 3 years. My approval is waiting on this only. Oh I know all about plantar fasciitis!!! Had it on both feet. Actually tore my ligament while having this. It takes about 6-12 months for it to go away. Went to see a foot doctor , tried meds, stretching. Nothing helped. The only thing that helped were these flip flops. My feet are fine today but I put theses flip flops on every day still. Just love them. I've been reviewing the postings here. They really help. Thanks to all. I did the initial doctor visit. All other appointments needed will be done by the end of the month. The office said they will submit paperwork to insurance by the end of the month so I assume maybe early May? Not sure. Any thoughts? Excited but nervous too.
