    My insurance (UHC) requires revised mental health evaluation for revision from lap band (2008) to sleeve (to be 2011). It consists of one hour talk and two hours of tests on first appointment. Then one more hour of talk two weeks later at second appointment. Then about a week to write a letter to surgeon for approve/not approve. Seems excessive. What was your mental health evaluation like ?
