I saw your inquiry and we would love to help you find the answer to your question. It looks like all plans for Kroger include benefits for Bariatric Surgery. But, the best thing to do if you have any questions is to call us toll-free at (800) 565-9140, or access your benefit information online at bcbst.com/blueaccess. BCBST highly recommends a pre-determination for this type of procedure. It’s basically sending us all the information ahead of time to make sure you meets the criteria to show it’s medically necessary to have this surgery. There’s a form online at http://www.bcbst.com/mpmanual/Bariatric_Surgery_Predetermination_Request_Form.pdf that you can take to your doctor and submit for that pre-determination. Pre-determinations usually take about 7-10 business days to review. Both you and your doctor will receive a response in writing. Hope that helps answer your questions.
