  1. My insurance company pays for it They are saying I need to go to the doc and get a cofirmation letter and have it faxed to them. The insurance has other requirements. I am so confused where to begin..Anyway Thursday I have an appointment with the GP.. I am hoping she knows the next steps. I thought someone here has already traveled thr redtape and could enlighten me. Good luck to you

I am not sure if it is with a GP or the selected surgeon or ????? For me the sooner I can have it the better I will like it.
