BCBS New York coverage?

I tried checking BCBS' site and can't find the info I'm looking for. Is anyone familiar with how it works in NY? I am on a PPO plan. I have over 6 month documentation from my nutritionist. Do they have a BMI limit?

Thanks for any info you can provide me with.
