Will PHCS cover?

My husband has PHCS insurance does anyone know if they often cover the lapband?

I have a BMI of 35.5 and i know the insurance wants a letter of necessity and a psychiatric evaluation. What kind of questions will I be asked during this evaluation? Any advise?
