Messa

Hello,

I am new to the forum and wondering if anyone that has Messa has just been banded. I plan to be shortly and this is what they are telling me I need. Letter from my primary care physician ( the hospital gave me a template for my Dr. to use) and an authorization letter from my surgeon.

Is this all that you needed or did Messa require something else?
