Insurance Question

Newbie here....Regarding insurance...my doctor's office said my insurance (BCBS IL) is now approving the sleeve, however, most company plans haven't caught up with the insurance company, and the sleeve is still being denied due to the companies not reviewing their policies and adjusting them to reflect the ins. company. Has anyone encountered this? If so, is there possibility of appeal? To whom does the appeal go...the company or the insurance company?
