BCBS FEP requires 3 visits with dietician/licensed nutritionist within 90 days. It says “prior to surgery” but does not say a time limit. Your old one may work but you should ask now because that first appointment with the nutritional counselors will start the clock ticking for your surgery unless your Dr has longer wait times. Other thing required by FEP Blue are: —Psych evaluation —BMI 40+ or 35+ with co-morbidities If I can figure out how To post pics on here I will post the page from FEP blue handbook.
