Did you submit your doctor visit with notes? I am in process now with UHC going through Optum too.

Does anyone know what they mean when they said we have to submit the Bariatric Resource Services weight loss form or UHC with chart notes? I’m not sure what kind of notes? Can my doctor type up a letter in regards to the chart notes? Cause my chart notes said discuss weight lost and out in a low carb diet. And the other didn’t even mention my diet.

I was vs on 9/06/12 @ 257 and I now weight 218 12/2/12 down 39 pounds!!! -
Me too! I had my surgery on 9/06 and was told its ok to have regular coffee and tea. They only had me on decaf coffee and decaf tea the first two weeks!

I was sleeved on 9/6 and was able to drink coffee the 2weel
