Vicki Loichinger reacted to DLCoggin in Dumping The Confusion About Dumping
Dumping seems to be a topic that generates a lot of confusion. Some believe that everyone has dumping following gastric bypass surgery. Some may experience it and not recognize that it is dumping. Others believe that the presence or absence of dumping affects the potential success (or failure) of bypass surgery. Still others are so concerned about dumping that they avoid trying new foods or even eating healthy amounts of calories and Protein. So let’s see if we can dump the confusion about dumping.

    Who Experiences Dumping

Estimates vary widely but somewhere between 15% to 50% of gastric bypass patients never experience dumping. It’s difficult to be exact because some experience dumping without realizing that their symptoms are the result of dumping. Others experience symptoms that they believe are the result of dumping when the cause is something else. Regardless of the exact number, a significant percentage of all gastric bypass patients never experience dumping.

    What Causes Dumping

There are still some unanswered questions regarding the causes of dumping but it is generally thought to be the result of food being “dumped” too quickly from the pouch to the small intestine. Food high in sugar or simple carbohydrates (aka “simple sugars”) can create a rapid increase in blood sugars (hyperglycemia). The rule of thumb is that white foods (e.g. potatoes, rice, Pasta, white bread, etc.) are usually simple carbohydrates. The sugar in the small intestine rapidly pulls Water out of the bloodstream and into the small intestine. A number of the symptoms of dumping including severe diarrhea can result.

With “late” dumping (see below), the body responds to the rapid rise in blood sugars with an increase in insulin to process the sugars. The increased insulin can cause the blood sugars to “crash” producing hypoglycemia (low blood sugar) and dumping symptoms follow. Symptoms may or may not include vomiting. I have experienced late dumping on several occasions, usually after eating fruit, such as grapes, or white rice, both rich in sugars. In my case, symptoms are shakiness and general weakness but I have never had any vomiting.

    What are the Symptoms

Just about everyone knows that dumping can cause vomiting. But all vomiting is not the result of dumping and all dumping does not cause vomiting. Symptoms of dumping include:

    · Nausea or queasiness

    · Profuse sweating

    · General weakness

    · Vomiting

    · Shakiness

    · Diarrhea

    · Faintness

    · Fatigue

    · Heart palpitations (or increased heart rate)

    Types of Dumping

Doctors divide dumping into two types and the symptoms may vary depending on which type you have:

    · Early dumping – usually occurs within 15 to 30 minutes following the ingestion of certain foods. Roughly two-thirds of people who experience dumping have early dumping.

    · Late dumping – usually occurs within one to three hours following the ingestion of certain foods. But it can occur as much as twelve hours after eating. About one-third of people who experience dumping have late dumping. Late dumping is linked with low blood sugar (hypoglycemia).

Success of Gastric Bypass and Dumping

There is no scientific evidence that patients who experience dumping are any more or less successful than patients who do not experience dumping. Dumping is not on the long list of factors that make gastric bypass surgery so effective for so many. So if you’re worried that your surgery might fail because you don’t have dumping, it just ain’t so!

    Fear of Dumping

This is a good news, bad news kind of thing. If fear of dumping helps you to make better food choices, it’s a good thing. But if fear of dumping prevents you from eating a healthy number of calories and getting ALL of your protein requirement, it’s a bad thing. Remember the Prime Directive – the bypass surgery is a tool but the gold medal is lifestyle changes. Fear of dumping (or anything else) is not sustainable, it’s not compatible with living and enjoying your life, and it’s not a realistic expectation. Lifestyle changes are about control, not denial.

So How Do You Avoid Dumping

    · Choose complex carbohydrates like whole grains instead of simple carbohydrates like white bread and sugary sweets.

    · No liquids for 30 minutes before and 30 minutes after eating. Pretty much a standard recommendation for all bypass patients but especially important for avoiding dumping.

    · Some people may not tolerate dairy products.

    · Eat smaller meals more often.

    · Keep a food diary to help you track your reaction to various foods. In addition to helping you avoid dumping, research has repeatedly shown that folks who maintain food logs lose more weight, lose it more quickly, and are more successful at maintaining their ideal weight.

    · Incorporate high Fiber foods into your diet. Fiber takes longer to digest and extends the time the food takes to move from the pouch into the small intestine.

So there you have it. Dumping is not pleasant. But it’s often easy to avoid with dietary changes. Dumping does not always involve vomiting; there are a number of possible symptoms. A significant number of patients never experience dumping. And dumping has nothing to do with long term success or failure of gastric bypass.

Stay focused. Stay active. Stay away from the scales. You’re gonna love the new you!!

You might try holding the capsule in your mouth for just a little bit so that it starts to soften, You may get a bit of the medicine taste in your mouth but not as bad as opening it and eating it was. I have avoided swallowing pills ever since I was banded seven years ago. Some I open up into crystal light and take an ounce of liguid with the medicine inside the caps. some I hold in my mouth for a bit. And for antibiotics I always ask for liquid. I feel like a little kid sometimes taking my PINK antibiotic. but oh well. Right now I have an abscessed tooth and I had the dentist give me liquid keflex, they had no problem. They just have to figure out the right dosing for an adult.

I don't work right now but I would keep a glass of Water on ice on my med cart and drink between each patient. Idk if you have a med cart or not. As for the Vitamins, take them before you go to sleep. Your body absorbs them better while you are sleeping because your peristalsis is slower.

I am not a nurse or work 12 hour shifts but have some surggestions. I always carry the bottle of Water with me no matter where I go and sip, sip, sip, sip. I know I have to drink 4 a day to get my 64 oz. of water. The Protein I either have that as a mid morning snack or I have it as Breakfast. I don't put lots of milk, I only use about a cup full, it makes the shake thick but it is less to drink. I can usually get in down in about a 1/2 hour. I find driving in my car I can get my fluids in much easier.

You know...its a funny thing about abuse. It happens at all levels and all incomes. Ironically, being in an emotionally abusive relationship hurt me worse than being in a physically abusive relationship. And my weight skyrocketed. I didn't eat excessively, but the stress prevented the ability to lose it. And guess what? High incomes don't mean we are rich...we can just be broke to a different level. You get chained to the mortgage, the car payments, the credit cards, and at the end of the week, you can still be living paycheck to paycheck.

Go have the surgery coz u deserve a better life. Don't listen to ur husband: put yourself and your benefit first. YOU DESERVE THIS. If he doesn't change his mind later, then you'll have decisions to make later. For now, focus on yourself and the better life you will enjoy.

Some like us to stay fat because it makes them feel superior to us. I don't think it really is because they worry about us. Our spouses don't want to have to compete with others. Our friends like that we aren't competition.

I wouldn't talk to them about it anymore. Most people look at us as fat lazy people looking for an easy way out and this is not the case. This is going to be hard work. I have not been open about it to some people. If you don't want people to know just tell them you're having your gallbladder removed. But again I wouldn't discuss it with anyone who has shown a negative response. Bottom line this surgery is for you and those you need to live for. Get on here everytime you are feeling down from those around you. it does help!

It is possible that he is afraid of you having the surgery and really doesn't know how to express it. But that would be trying to see some good in this. But he sure is acting like a jerk right now.
As hard as it is try not to be to discouraged. You have the date, have the surgery, gather someone to support you and heal and then see where things are.
Emotional abuse is so bad. I know I have been there.

Vicki Loichinger got a reaction from Cheryl Barfoot in Revision From Band To Rny And Why.
I am in the process of insurance approval to have a revision from Band to RNY. I have had my band for seven years. I have been very lucky to have not had any complications, no slips ect. My band is unfilled now, because i never did get the 'sweet' spot, either to tight and things would come back up or I would revert to soft high carb foods, or too loose and I can pretty much eat anything. I had my surgery for the band in Cincinnati Ohio, (I live in Indianapolis) The surgeon Dr, Trace Curry was excellent,
I do think traveling back and forth for fills ect did hinder me getting where I needed to be for my fill to be right. But no matter what the band was not going to help most of my medical issues.
Since the band I have lost and gained the same 30 to 40 pounds. Now at my heaviest. And the band did not help any of my medical problems. I am hoping the RNY both with the smaller stomach and mal absorption and the actual surgery itself will help with my uncontrolled type 2 diabetes and insulin resistance. I am so sensitive to insulin that I swell and ache all over but I have to take large amounts to keep my blood sugar down. It is like I am on a merry go round, hurt, can't exercise, can't exercise, gain weight, blood sugars out of control, more insulin, can't breath because of severe asthma and copd, need steriods about once a year, higher blood sugars, more weight gain, can't exercise, and around and around we go
My hope and prayer is that this surgery will add years to my life that I can spend with my children and grandchildren. I need to GET a life, because what I am doing right now is not living.

You are so right about that. My mother keeps bringing up people that had the surgery and wasn't successful at losing or keeping the weight off. I guess she is not being evil, lol. I know there is a support group so I think I'll call my nutritionist and find out the days and times. My manger at work has been great support. She had the lap band 5 years ago and looks great. Our insurance pays for the skin removal around the belly so she just had that done. Vicki you are great!!!

I never looked at it as if they are scared. My mom said some mean things a few weeks ago, when I asked for Thanksgiving dinner in October. Maybe she said it to discourage me from having the surgery. I will have to remind myself they have some fears too.

I have been using a CPAP/BIPAP for about 13 years. It does take a bit of getting used to. But once you do it will be your new best friend. A couple of suggestions. Try laying down for a short nap with it. Not so much mental pressure on having to sleep all night. But do put it on every night, you may pull it off in your sleep but continue to put it on every night. You may also need to have the pressure adjusted. Call your sleep doctor. You may also need a different headgear and mask or the one you have is not adjusted properly. Call the provider of your machine and mask and ask if they can help you in a better fit. It is worth going to what ever measures you can to get the proper adjustment both in the pressure and the fitting of the mask. It will make a real difference in how you sleep and how you feel the next day.
And after your surgery theses needs will need to be addressed again as your sleep apnea may improve and settings may need to be changed again.
Please keep trying to used the mask, and get the answers and help you need. Just like the WLS it is not a one size fits all. WIshing you the best and feel free to ask me any questions you have,
