    Second times a charm (I hope)

    I am currently three years post sleeve sitting ten pounds heavier than when I stared contemplating the SADI revision surgery. I am pretty hopeful and excited this time though. I am already eating better, exhaustively researching the procedure and the future diet. Finally I found a therapist that specializes in disordered eating near me who is taking new patients. It wouldn’t be possible without telehealth because she’s a couple hours away (something positive that came from COVID). Within 15 minutes tonight we discussed my mental health, medications, medical history, potential surgery, a change in the dietician and touched on some of my emotional eating concerns. She thinks she can help me and she asked me to think about it and talk it over with my current therapist and if we agree it’s a good idea she wants me to sign a release so that she and my therapist can work together. I feel so excited and hopeful that this is going to make all the difference this time. I am curious how long the process usually takes. I’m hoping I will have a pretty good idea of how much help I need and know whether I am ready to move forward by the time my surgery date rolls around.

    Almost 3 years out!

    Good Evening All! I am almost 3 years postop from my Duodenal Switch w/ Biliopancreatic Diversion. It has been a long 3 years but it has been worth all of the effort I have put into this. I recently had to have emergency surgery to fix a wall hernia/bowel obstruction and something with my mesentery. I am scheduled for my excess skin removal on July 8th 2024. I am excited and I am proud of myself for all of this hard work. Its not easy with this surgery and anyone who has gone through it or is about to, remember one day at a time and YOU CAN DO THIS!!

    Now that I'm in maintenance mode, I'm getting a into a routine for my meals. Every day, I start out with 8-16 ounces of water, and then a proffee, which I have come to look forward to even the night before. My proffees are simply a black coffee with a protein powder added. There are three products that I cycle through: Premier Vanilla, Orgain Vanilla, and Dymatize Vanilla. For second breakfast on workdays, I will have a low-fat yogurt with two tablespoons of PBFit and two teaspoons of no sugar added dried cherries. I will have ingested 35-45 grams of protein at this point between the two breakfasts, with 250-285 calories, and about 20 carbs. For second breakfast on non-workdays, I will prepare two servings of plain, instant oatmeal with a tablespoon of an olive oil-based spread. This means I will have had 34 grams of protein, 365 calories, and 38 carbs. Non-workdays are when I am being very active with training sessions, so I allow myself more carbohydrate fuel. Snacks on any day are always mixed nuts, even when I am travelling. I will have 0.2 cups of a blend that I make myself. It consists of dry roasted peanuts, cashews, pumpkin seeds, sunflower seeds, pistachios, and Brazil nuts. This is 5 grams of protein, 163 calories, and 7 carbs. Breakfast and snacks have been the easiest to nail down. Lunch and dinner have more variables, and I prepare enough for leftovers. I concentrate on protein first, and then add vegetables. Typically tempeh, tofu, or Field Roast products with roasted or sautéed vegetables. Today, I will be eating leftovers from last night. Two ounces of tempeh with four ounces of roasted vegetables that consist of red and yellow sweet peppers, sweet potatoes, small purple potatoes, zucchini, and carrots. I will add a tablespoon of olive oil-based spread, break up 3 walnuts to sprinkle of top, and garnish with two tablespoons of grated Parmesan cheese. This particular meal will be 19 grams of protein, 377 calories, and 28 grams of carbs. Bear in mind that I do eat more carbs when I am not working, and I focus on ingesting healthy carbs instead of breads/crackers/chips/crisps. It's a helluva journey and I'm thankful to be on it!

    I had the Pho broth from week one, I made it myself so it was not too salty or chilli hot. It was so good I could have bathed in it. At 2 years + now, I can manage a couple of spoons of noodles at the end of the meal. I don't crave mountains of noodles now, I am used to just a few for the mouth feel.

    What to take to hospital

    You will be wearing a hospital gown and given non-slip socks, so all you really need is to wear something to the hospital that is comfy with a stretchy or loose waist you can wear home, and bring clean undies and socks. Also bring a toothbrush, travel toothpaste, hair brush, deodorant, chapstick, and lotion. Don't bring meds or fluids as they will need to administer those to you and record them in your chart. But check on this because if you are taking something unusual, they may want you to bring it. In my case, they just gave me my daily thyroid medication from the hospital pharmacy. Bring your phone and charger and a book, maybe headphones if you'd rather listen to music. Hospitals are kinda gross, so the less you bring, the better. I had an incident where my IV came unattached and I ended up with watered down blood everywhere, which would have ruined any pillows or blankets from home. Oh, I brought a small pillow for the car ride home that is made for abdominal surgery (search Amazon for hysterectomy pillows) and can velcro to your seatbelt for extra comfort for your incisions. It was nice but not 100% necessary. For home, I had a weighted heating pad that I loved for sleeping. You will also want a bottle of liquid adult strength Tylenol at home for pain.

    Thank you NickelChip for these ideas. I'm also having trouble. I asked my surgeon if my sugar addiction was going to be a problem after surgery and he said "no". Since I had the sleeve, I don't get dumping syndrome when I eat sugary things. I start over every morning, every afternoon, and every evening - lol. Because of back pain, I hadn't been able to walk very often, but I've been trying to get around the neighborhood a 1-2 times a day on the days I work from home. I want to get serious with weight loss so that my pain will lessen. I know sugar is an inflammatory agent - but, sugar addiction is real. brandycsiz we can do it!

    How can I possibly live like this?

    Kinda flamboyant delivery. Also sniffs and rubs his nose a lot. I would watch Dr Matthew Weiner's videos in preference because they seem more fact/research based rather than opinion/experience I think but no harm mixing it up once in a while so thank you!

    It is consistently amazing to me how seemingly small weight shifts can cause noticeable changes in the body! You are looking great!! Have you done measurements? Those are really cool to watch, especially in the first months...

    Weight Gain Scare

    Not that uncommon. I can eat a pretty "normal" amount of food as well, depending on what time of day it is, activity level and what kind of food. Take a look at Dr. Weiner's video on increasing portion sizes: Interesting chart about 1:30 min in. I wish surgeons would educate their patients on this and telling them that it's normal and nothing to be scared about ("my surgery failed" - no it most likely didn't).

    Sadi is so lonely

    One site says that the gas and more frequent/ loose bowel movements are a little less than the DS but definitely an issue with the SADI and the gas can be quite stinky as well which is probably the biggest fear I have right now aside from the real scary risks. I THINK the bathroom issues are just when you eat bad and if that’s the case that may actually help keep me on track because I obviously don’t want to have those issues. I have an appointment tomorrow morning with my family PA and that’s definitely a question I will be asking her if she knows. I think the supplement question is more likely going to be one for the surgeon but I will ask her about that as well. I have until the end of next month when I meet with the surgeon to come up with a list of questions for him (whatever I can’t get answers for before then). Thanks again and please Let me know if you think of anything else I should ask. Until yesterday morning hadn’t even heard of this procedure. At least I have a list of questions to start with now.
